Tips for Completing your GenSol Swab Collection Sheet

Here at GenSol we realize collecting DNA from several dogs can be a challenge. Most of our customers cautiously collect their samples, following our sample collection video, separating dogs, refraining from food, water and nursing, etc. What some folks don’t consider is that just as important for accuracy and efficiency, is completing the swab collection sheet.

GenSol Transcriptionists work diligently to transcribe your information from the collection sheet to our vast database of customer and canine genetic information. This information is also used to identify your sample, create your certificate of results, and using your email address, send your results to you, the customer who is anxiously waiting for results. If there is an issue with your paperwork, a clearly printed, current, accessible phone number with a working voicemail option, is the lifeline to correcting the error and getting your sample back to the lab in a timely manner.

Here are some tips to help our customers in completing this form as accurately as possible:

Tip #1 Read ALL information on the front of the Swab Collection Sheet BEFORE completing it.

Tip #2 Print CLEARLY

Tip #3 The Owner’s Name on the front of the collection sheet, will be the name printed on the certificate.

Tip #4 Your phone number is vital information in the event the lab has a question regarding your paperwork or sample.

Tip #5 Your email address is where your results certificate will be sent.

On the backside of your Swab Collection Sheet:

Tip #6 PRINT CLEARLY This information will be entered as written, on your results certificate.

Tip #7 Double check your registration number for accuracy.

Tip #8 Don’t abbreviate your dog’s breed.

Tip #9 WRITE your sample ID sticker # next to Sample ID #. The sticker is for sealing your swab, not to be placed on your paperwork.

Tip #10 (Which should be Tip #1) CIRCLE YOUR TEST!!! The lab does not access your order history. At this time, the test you circle, tells the lab what test you are requesting. If the test is not circled, it will delay your results.
